The Community Connector Service builds on the work of Social Prescribing and Village Agents across the county. This new exciting service seeks to make use of the strengths in the local community by working closely with the voluntary, Statutory Sector and connecting individuals to services that can support them
The successful applicant will meet individuals and help them to identify their health and wellbeing needs, you will also help them to locate local resources in Stroud area that can help them to achieve their identified goals
Community Connector will gather information about local groups, services, activities that individuals can be signposted to
You will work in various settings across Stroud including GP surgeries, community centres, Redwood House, as well as venues you will identify for yourself
The successful applicant will need to be experienced in delivering Person Centred and strength focused services that enable and empower individuals to build links and networks within their local communities.
You will carry an agreed caseload Community Connector will need to be ‘goal and outcome focused’. You should have the ability to offer a highly flexible approach to the customer within your working hours to cover a contract that includes occasional evening/weekend work according to need
